To qualify for participating in PACE program, an individual must:
The prospective participant will be scheduled for an assessment with the rest of the care team within a very short time. The State Administering Agency must assess the potential participant, including any individual who is not eligible for Medicaid, to ensure that he or she needs the level of care required under the State Medicaid plan for coverage of nursing facility services, and that he or she meets all requirements for PACE eligibility. The criterion for determining if an individual is able to live safely in the community is established, and must be approved, by the State. Once the assessment by the PACE team is completed, a plan of care will be created for the participant.
